God's Chosen Ministry Community Outreach is a faith based organization that empowers individuals to reintegrate into communities successfully. The Ministry serves veterans and their families, formerly incarcerated men and women and their families, and individuals experiencing issues related to homelessness, ID's/Driver license assistance, birth certificate, social security cards, mental illness, and financial responsibility. Services include mental health counseling, substance abuse treatment, life skills enhancement and homeless prevention. The Prisoner Reentry Transition Program assist incarcerated individuals with a successful transition to their community after they are released. God's Chosen Ministry aims to build safer and stronger communities by empowering individuals to lead productive lives. Additionally, in collaboration with other organizations, they offer spiritual wellness, fitness and aerobics, yoga, bullying prevention and self-defense training, youth development, job and employment, soft skills, food and clothing assistance and rental and utility assistance.
