Charlotte Community Health Clinic’s Formerly Incarcerated Transition (FIT) Program

Charlotte Community Health Clinic (CCHC) is a nonprofit organization and Community Health Center that serves Charlotte. They accept patients with Medicaid, Medicare, most private insurance, and those without health insurance. In collaboration with the Center for Community Transitions, CCHC launched the Formerly Incarcerated Transition (FIT) program. People released from prison with chronic diseases, mental health issues, or substance use disorders sometimes struggle to get affordable medical care. Even before their release from prison, the FIT Program connects them with appropriate health care treatments and other reentry resources. FIT is a project of LifeWorks that provides individuals with a criminal record with the resources and tools they need to achieve their professional and personal goals. It employs Community Health Workers with personal incarceration experiences to assist clients in developing a complete reentry strategy. The FIT project also receives private funding for a voucher system, which will provide vouchers to formerly incarcerated individuals to be used to pay for clinic visits and prescriptions at local care providers.
