San Joaquin County Clean Slate Program

The San Joaquin County Clean Slate Program was established to assist formerly incarcerated individuals with barriers to reintegration. Staff are specially trained to offer targeted support for justice involved persons who may struggle to obtain gainful employment, secure stable housing, take advantage of educational opportunities and enroll in public benefits due to their criminal convictions. Individuals seeking support from the San Joaquin County Clean Slate Program must complete and file two separate forms. The Financial Declaration requests information regarding marital status, employment, all sources of income and all sources of outstanding debt. Additionally, individuals must complete the Clean Slate Intake form. This form records information regarding an individual's status with the court, including supervised release, parole and probation. Individuals must also disclose whether or not they are facing pending charges and note any time spent in correctional facilities. The office will use these forms to determine eligibility and identify the best assistance available for each individual.
