Grace House

Grace House is a Connecticut-based residential re-entry program for women who have been impacted by the criminal justice system. It is a program run by the Perception Programs, Inc. which is a nonprofit organization that promotes wellness through holistic and behavioral health care for individuals and our community. Re-entry services at Grace House include substance use and mental health assessment; individual and group counseling; medication management; and medication. The services help women in building essential skills for successful community reintegration. Grace House is trauma and gender-informed, and all treatment is tailored to the unique needs of women who have experienced trauma. The program includes 20 hours of individual and group therapy each week. The treatment is focused on relapse prevention, healthy relationships, life skills, and mood regulation. The case managers at Grace House assist residents with case management, risks and need assessments, aftercare planning, employment, procuring IDs, medical appointments, housing, and discharge planning.
