Roots Community Health Center

The mission of Roots Community Health Center is to empower those affected by systemic inequities and financial insecurity. The do this through medical and behavioral health services, workforce assistance, housing services, outreach efforts, and advocacy. Roots Community Health Center envisions a country where Black communities thrive through self-sufficiency and self-determination. In addition to their medical centers, Roots Community Health Center offers facility and home-based care. The goal of the program is to ensure access to primary care for individuals in transitional housing, recovery centers, and other situations which prevent regular access to medical care. They currently serve the greater Bay Area and are working to expand to other areas of California.
