Bridges to Health is a community clinic providing primary medical care services to underserved and uninsured individuals. While the clinic is not equipped to offer management for serious illnesses or dental services, staff will coordinate with local resources and assist individuals with obtaining the care they require. All new patients of Bridges to Health must meet qualification criteria outlined by the organization. Individuals must prove they are residents of Grant County and that they are currently unable to obtain insurance, including Medicare and Medicaid. Individuals must also live at or below 300% of the federal poverty level. New patients must submit an application for service and receive acceptance prior to scheduling an appointment. All individuals must schedule appointments before visiting the clinic, as hours vary each week and practitioners will not see walk-in patients.
