Acenda Bridge Program

Acenda is a nonprofit organization that offers life enhancing services to individuals, families, and communities throughout the state of New Jersey. Acenda's Bridge program develops plans that address the needs of incarcerated individuals within the Seriously and Persistently Mentally Ill (SPMI) population. Program participants are provided with an opportunity to receive mental health and non-mental health services once released from a correctional facility. Program staff works in collaboration with the Gloucester County Jail staff and Community Providers to create a seamless transition from the jail to the community. A pre-release plan is developed during the period of incarceration to outline the clients needs during the reentry transition. Coordination of those identified services start prior to release to allow immediate needs to be addressed upon release. These case management services continue beyond the incarceration and just-released phase for approximately 3 to 6 months.
