Dakota County Reentry Assistance Program (RAP)
Description
The Dakota County Re-Entry Assistance Program is a state sponsored program that serves individuals exiting Dakota County Jail. The program assists individuals in the jail with histories of multiple jail stays to reduce the likelihood that they will return to jail. The program also offers transitional and support services that helped individuals who are exiting the Dakota County Jail with their return to the community. Community Corrections reviews applications from incarcerated individuals who are requesting reentry support, facilitates interviews to identify eligibility and determine service needs for incarcerated individuals selected for the program. After an applicant is approved for services, staff identifies and arrange support needs and helps to coordinate a reentry case plan. Once the individual is released into the community, the next step is to help with their transition back into the community, and to offer onoging support to the inmate for up to 90 days after release.
