First Nations Community HealthSource

First Nations Community HealthSource (FNCH) is a community based health center in Albuquerque, New Mexico. First Nations offers healthcare services that address the physical, social, emotional, and spiritual needs of the community members served. Provided health services are accessible, affordable, and available to underserved and unserved individuals and families, including those who may be experiencing homelessness. First Nations operates two clinic sites and four school-based health centers. In addition to health services, the FNCH Youth Mentoring Program (YMP) offers children individual and group mentorship services that promote youth development. Services include boosting academic achievement, attendance, and self-esteem levels. The goal of this program is to make a difference in the life of children by serving and supporting those involved in the juvenile justice system. The FNCH Social Services Program assists patients with housing, economic, cultural, subsistence, transit, childcare, and other healthy lifestyle issues.
